How to Become a New Patient
To Get started, follow the steps below:
Our process is simple
Step 1
Click on the Contact Us button below to request a new patient appointment.
Step 2
You will receive a phone call from the Doctor within one (1) business day to schedule your appointment and receive your New Patient Information packet.
Step 3
Bring your paperwork with you OR email it back ahead of your visit for the doctor to review. If you want someone to accompany you with your visit, that is fine as well.

EVALUATION
(Fee is $600)
Evaluations are then done face to face for 2 hours but may be broken into 2 one-hour appointments if desired. Visits are available Monday through Thursday.
FOLLOW UP
10-20 minutes - Fee is $150
21-35 minutes - Fee is $200
36-50 minutes - Fee is $250
51-65 minutes - Fee is $300
These appointments can be done by telemedicine (virtual visits) or face to face and are a variety of minutes in duration depending on need.

There may be a $100 fee for not canceling 48 business hours (M-F) before visits.
